This policy explains how Made For More Wellness Co. (“we,” “us”) handles information collected through this website, madeformorewellnessco.com. It covers the website only. How we protect your health information as a patient is described in our Notice of Privacy Practices, which is available at the clinic and from our team on request.
Information you give us
Contact form. When you send a message we receive your name, email address, optional phone number, the service you’re interested in and your message. The form is delivered to our inbox by Web3Forms, a form-processing service. Please do not include medical details or health history in the form; we’ll discuss those privately with you at the clinic or by phone.
Hormone quiz. The quiz runs entirely in your browser. Your answers are not sent to us or stored anywhere.
Chat assistant. The chat answers common questions about our clinic. We do not store chat conversations. If an AI service is enabled for the chat, your message is sent to that service only to generate a reply. Please don’t enter personal health information in the chat.
Online booking and patient portal. Appointment booking and the patient portal are provided by our practice-management system on a separate website (md-hq.com) under its own privacy and security terms.
Information collected automatically
We use Google Analytics to understand how visitors use the site (for example, which pages are viewed and whether a call or booking button was clicked). Google Analytics uses cookies and collects data such as your device type, browser, approximate location and pages visited. We do not send your form contents, quiz answers or chat messages to Google Analytics. You can opt out with the Google Analytics opt-out browser add-on.
Embedded Google Maps on some pages may set cookies from Google when the map loads.
